One page to grab on the way to the hospital

If you care for someone, there is a version of this evening where you are in a car park at 11pm being asked what medication they take, and you cannot remember.

This is one page. Fill it in once, put it on the fridge, put a copy in your bag. It is free, there is nothing to sign up for, and you can print as many as you need.

Fillable PDF

Type it on screen and save. Best if the medication list changes often.

Print and write

Ruled for handwriting. Best for the copy that lives on the fridge.

Free, with no email address and no account. Print as many copies as you like, including for other people.

What is on it

One side of one page, holding the things a paramedic, a nurse or a relative asks for first.

  • Who they are, and who you are to them
  • The conditions that matter in an emergency
  • Allergies and reactions
  • Current medications, with dose and timing
  • The doctor, the pharmacy, the preferred hospital
  • Who to call, in what order
  • Where the important documents are kept

Print it properly

Print at 100% or Actual Size, not “Fit to page”. Fit shrinks the margins and can clip the lines you write on.

Anyone caring for a parent, a partner or a friend is welcome to this. So is anyone who has ever been asked “and what does she take?” and heard themselves say “I’d have to check.”

Questions

Do I have to give an email address?

No. There is no form, no account and no list. The link downloads the file.

Can I hand these out?

Yes. Print it, photocopy it, leave a stack of them out. If you run a support group, a library, a day centre or a practice you do not need to ask. Message the studio through Etsy if you would like a version without the studio name on it and one will be sent over.

How should I print it?

At 100% or Actual Size, not Fit to Page. Fit shrinks the margins and can clip the lines you write on.

Is this medical advice?

No. It is one page to write things down on. It does not replace anything a doctor or pharmacist tells you.

This sheet is an organisational tool. It is not medical or legal advice, and it does not replace anything your doctor or pharmacist tells you.

If you want the fuller version, there is a caregiver binder and an emergency binder — but this page is free and always will be.
